Deleting those files from the AppData\Roaming\Microsoft\Windows\Recent\AutomaticDestinations folder and restarting Explorer often resolves the issue.
Another thing to confirm is that you’re not in a restricted environment, Group Policy or registry settings can disable pinning to Quick Access. If this is a work device, your IT administrator may have enforced such policies. On personal devices, you can check by opening Local Group Policy Editor and ensuring “Do not allow pinning items in Quick Access” is not enabled.
If the issue persists, try creating a new Windows user profile temporarily. If Quick Access works there, it confirms the original profile is corrupted and may need repair. Also, make sure you’re running the latest cumulative updates for Windows 11, as Explorer fixes are often included.
